Horizon Medical Center focuses on the needs of the persons served and practices a unified, interdisciplinary approach to patient care. Our team is made up of highly specialized doctors, therapists, nurses, and case managers dedicated to providing expert care of people with a wide array of injuries.

Our team of professionals provides caring, comprehensive, intensive physical rehabilitation to the person served. Rehabilitation services are structured to enhance the recovery efforts of the person served either at the bedside or in the therapy gym. The goal of the program is to enable patients to function at their highest possible level of independence.

In accordance with Title VI of the Civil Rights Act of 1964 and their implementing regulations, Horizon Medical Center will, directly or through contractual or other arrangements admit and treat all persons without regard to race, color, creed, religion, sex, sexual orientation, age, marital status, disabilities or natural origin in its provision of services and benefits, including assignments or transfers within the facility and referrals to or from the facility.

Convenient off-street parking designated specifically for disabled persons.

Curb cuts and ramps between parking areas and buildings.

Level access into the Hospital.

Fully accessible offices, meeting rooms, bathrooms, public waiting areas, cafeteria, patient treatment areas, including patient rooms.

A full range of assistive aids is provided to persons with impaired hearing, vision, speech, or manual skills without additional charges is provided to persons with impaired hearing, vision, speech, or manual skills, without additional charge for such aids.
