Who we are & What We Do
Our commitment to superior quality care and personalized advancement at Horizon Medical Center is made possible by our skilled team of rehabilitation specialists, support staff, and administrative personnel. Depending on your diagnosis, rehabilitation needs, and goals, your team may include:
- Physiatrists are physicians board certified in physical medicine and rehabilitation and will direct your overall medical care. An internist, neurologist, cardiologist, or other medical specialist may also be involved in your treatment as needed.
- Rehabilitation Nurses are on duty 24 hours a day to assist you with medications and self-care and help reinforce techniques learned in therapy sessions.
- Physical Therapists will optimize your functional movement, muscle control, balance, and flexibility to build strength and endurance for mobility and daily tasks. Their primary goal is to help you improve your movement, manage pain, and enhance your overall quality of life.
- Occupational Therapists will help you regain the skills and strategies needed for daily living and work. Their primary goal is to enable you to participate in activities that are meaningful to you, despite any physical, mental, or emotional challenges you may face.
- Speech-Language Pathologists will work with you to improve language, cognition and communication skills, as well as swallowing deficits. Their primary goal is to support your ability to communicate effectively and safely.
- Respiratory Therapists specialize in the management of breathing disorders and respiratory conditions.
- Neuropsychologists offer counseling to help you understand and cope with the many changes you will experience both during rehabilitation and after discharge.
- Case Managers will collaborate with our physicians and therapists to coordinate your discharge, helping to plan for the appropriate destination, continued medical and rehabilitation services, and equipment that you may need. They will work closely with you and your family to ensure that your rehabilitation and discharge needs are met.
- Pharmacists will work closely with you, your physician, nurses, and therapists to address your medication needs and support your full engagement in the rehabilitation process.
- Dietitians will provide a nutritional plan designed to help you regain your strength. This may include the need for any supplements or texture modifications as well as other special dietary needs.
